Why Am I Not Losing Weight on GLP-1?

You started your GLP-1 journey full of hope. You were told this was the medication that was going to change everything. And maybe it did — at first. But now the scale isn't moving and you are left wondering what went wrong.
First things first — you are not alone. This is one of the most common questions I hear from patients at Concierge Medical Clinic. And the good news is there is almost always a reason — and a solution.
Let's talk about why GLP-1 medications sometimes stop working and what you can do about it.
WHAT IS A GLP-1 MEDICATION?
GLP-1 receptor agonists like Semaglutide work by mimicking a hormone in your body that regulates appetite, blood sugar, and digestion. They help you feel fuller faster, reduce cravings, and slow down how quickly food leaves your stomach.
But they are not magic. They are a tool. And like any tool they work best when used correctly.
REASONS YOU MAY NOT BE LOSING WEIGHT ON GLP-1
1. Your Dose May Need Adjusting GLP-1 medications are typically started at a low dose and gradually increased. If you are not losing weight it may simply mean your current dose is not yet therapeutic for your body. This is completely normal and is exactly why having a provider monitor your progress matters.
2. You Are Not Eating Enough Protein One of the biggest mistakes GLP-1 patients make is not prioritizing protein. Because your appetite is suppressed you may be eating very little — but if that little bit is not protein rich you will lose muscle instead of fat. Aim for at least 80-100 grams of protein per day.
3. You Are Not Eating Enough — Period This sounds counterintuitive but it is true. If you are eating too few calories your body goes into survival mode and holds onto fat. GLP-1 reduces appetite significantly — make sure you are still fueling your body with enough nutrients even if you are not hungry.
4. Hidden Calories Are Sneaking In Liquid calories from coffee drinks, juices, alcohol and smoothies can add up quickly without you realizing it. Even healthy foods can stall weight loss if portions are not monitored. A simple food journal for one week can be very eye opening.
5. You Are Not Moving Your Body GLP-1 medications work best when combined with regular physical activity. You do not need to run marathons — even 20-30 minutes of walking daily can significantly boost your results.
6. Stress and Sleep Are Being Ignored High cortisol from chronic stress and poor sleep are two of the biggest hidden saboteurs of weight loss. When your body is stressed it holds onto fat — especially around the midsection. Prioritizing sleep and stress management is not optional — it is essential.
7. You May Have an Underlying Condition Thyroid disorders, insulin resistance, PCOS and other metabolic conditions can make weight loss significantly harder even on GLP-1 medications. If you have been on a therapeutic dose for several weeks with no results it may be time to dig deeper with lab work.
8. Your Body May Have Plateaued Weight loss plateaus are completely normal and happen to almost everyone. Your body is smart — it adapts. When this happens it may be time to reassess your plan, adjust your dose, or make changes to your nutrition and exercise routine.
WHAT SHOULD YOU DO?
If you are not seeing results on GLP-1 the worst thing you can do is give up. The best thing you can do is talk to your provider.
